d. Your contact information, including your name, address, telephone number, and email address.

e. A statement that you consent to the jurisdiction of the courts in Bangladesh and that you will accept service of process from the person who submitted the original copyright infringement notification or their agent.

f. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.

If a valid counter-notification is received, we will review the information provided and may restore the removed material, unless we receive a court order restraining us from doing so.
